Community Healthlink's Community Support Program (CSP) provides short term, intensive outreach, and case management services to support individuals at risk of repeated psychiatric hospitalizations and/or inpatient substance abuse treatment admissions. CSP services are flexible and based on individual needs but focus primarily on helping clients overcome challenges with establishing and maintaining access to health care, community-based resources, and social/peer supports.

CSP Case Managers meet with referred clients in a variety of community settings, complete intake assessments and person-centered service plans, then assist clients with achieving the goals identified in those plans. CSP- duties include researching community-based resources and assisting clients to connect with these resources by helping with things such as short-term transportation, advocacy, encouragement, skills building, coaching, and completing paperwork.

Responsibilities:

  • Develop and maintain rapport with individuals referred to CSP.

  • Provide telephone outreach and home/community visits if needed. Provide "hands-on" case management support to clients, including providing short-term transportation to support engagement with providers and social services. Link individuals to resources and valuable information to improve health and wellness.

  • Help advocate for individuals to improve their access to care.

  • Complete required written documentation per program expectations.

  • Document in HHMIS System

  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.

Qualifications:

  • Grade C04: Case Manager II: Requires a Bachelor's degree;preferably in the field of psychology or human services . Strong communication and listening skills.

  • Knowledge of substance use disorder, mental health and related issues, familiarity with community resources and entitlements.

  • Strong analytical skills, including the ability to recognize problems, think creatively, and devise innovative solutions.

  • Effective communication and listening skills.

  • Comfortable working independently and within a group.

  • Flexible yet organized.

  • Bilingual (Spanish/English or Vietnamese/English) a plus.

  • Must have a current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work related purposes including transporting clients in your own vehicle.
