Job Description

Summary

Join us in the Care Ecosystem, a telephone-based support, education, and care coordination program for patients living with dementia and their caregivers.

  • Join us in the Care Ecosystem, a telephone-based support, education, and care coordination program for patients living with dementia and their caregivers. The goal is to improve the health and wellbeing of patients with dementia and their caregivers, as well as to decrease unnecessary medical costs. The Care Ecosystem is designed to supplement primary care by providing education, links to community-based services, behavioral management and advance care planning support. The Health Coach is the primary point of contact for patients and their families enrolled in the program, collaborates with their health care providers, and experiences regular consultation and guidance from a multidisciplinary clinical team with expertise in dementia. This position is 2.5 days/week, daytime hours, weekdays only.

  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: High School Diploma preferred. Bachelor's Degree preferred.
    • License/Certifications: Valid Maine Driver's License. Health Coach certification preferred.
    • Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
    • Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
    • Bilingual preferred.
    • Sensitivity to cultural and ethnic diversity preferred.
    • Local community resident with good knowledge of the resources available in the community preferred.
    • Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
    • Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
    • Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
    • Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
    • Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
